Teen Volunteer Application

THIS IS NOT A SUMMER PROGRAM. Requirements for the Year Round Teen Volunteer Program are as follows:

* Provide Proof of immunizations including: MMR and Varicella

* Commit to volunteering for 4 CONSECUTIVE MONTHS

* Commit to volunteering one 3-hour shift each week (same day and time each week)

* Attend a new volunteer orientation and interview

* Purchase a Volunteer Shirt ($10.65)

* Have a sincere interest in helping others in a caring compassionate and dedicated manner

* Have a professional and positive attitude with the ability to accept supervision graciously
